I, Oliver Green, am the parent and legal guardian of Noah Green, born on September 12, 2016. Due to unavoidable circumstances, I am granting temporary guardianship of my child to Emily Parker, residing at 234 Birch Road, Seattle, WA, 98102, specifically for the purpose of school enrollment and related educational responsibilities. This guardianship will be effective from January 1, 2025, to December 31, 2025.

During this period, Emily Parker is authorized to enroll Noah Green in school, complete necessary registration forms, communicate with school administrators and teachers, attend parent-teacher conferences, and sign any documents required for Noah’s participation in school activities and programs. Emily is also authorized to address any educational or disciplinary matters on behalf of Noah.
