From the name itself, a medical report is a written document on the results of a medical examination conducted on a patient. It typically describes the findings of the given individual. This could be on the history of a patient’s medications and records of medical conditions.

The first part of a medical report should include a patient’s personal data. This typically includes a person’s name, address, date of birth, gender, and contact details. How to Write a Medical Report for a Patient

A patient’s medical report is based on the results from the various tests or examinations conducted. To write the report, it’s best to use proper wording that a reader may understand.

Keep in mind that the people who may get a hold of the report may or may not be a part of the medical field. The report should contain a brief but understandable executive summary of the actual result. The details should also be based on actual facts. Try not to make it too complex as it can possibly be interpreted differently.

Finally, present the details in a chart or a paragraph. It doesn’t have to be lengthy but you still need to make it informative. After all, a person is only interested in knowing what’s wrong and what can be done.

Guidelines for Writing a Medical Report

  • Make it detailed. Provide information that is significant. This could be the causes of such result, the seriousness of a situation, or a thorough discussion on the current state of a matter.
  • Organize the report chronologically. Providing a timeline of events may provide a clear image of a situation.
  • Keep a personal copy. Although this may not be entirely necessary, most clinics like to keep medical reports for future references.
  • Maintain professionalism. It’s highly advisable to have the report typed and printed. Once this is done, sign it for authenticity.
