How Do You Write an Employee Incident Report?

An employee’s report of tips to employer is a tax form provided by the Internal Revenue Service for employees to keep track of tips received regularly.

These tips can come in the form of cash, debit or credit card adjustments, and the like. The tip total received by an employee must then be reported to their respective employers for tax purposes.
