What Is a Questionnaire?

A questionnaire example is a tool for data gathering and research that consists of a set of questions in a different form of question type that is used to collect information from the respondents for the purpose of either survey or statistical analysis study.

How to Create a Questionnaire?

  • Determine what you want to learn from conducting your questionnaire.
  • Think of questions that will help you get the information you needed.
  • Use a close-ended question to gather more specific answers and use open-ended questions to gather feedback.
  • Write clear and simple questions to avoid confusion and bias.

The Importance of Questionnaire Structure?

The most common structure of a questionnaire is that the open-ended questions are placed at the first section of the questionnaire. It is not bad but it is not good either. You see, open-ended questions tend to require more thought and effort and takes more time in answering than any other type of questions.

There may be a possibility that respondents can not answer in detail, they need to be warmed up. In this case, easy to answer questions must be placed at the first section then the questions that require more thought can then be placed at the end of the questionnaire. The respondents are warmed up and the response of the questionnaire will be better.

What’s the Difference between Questionnaire and Survey?

A questionnaire, as stated above, is a set of written questions used for collecting information that is beneficial to a single individual. The respondents of a questionnaire are not aggregated for analysis.

The survey, on the other hand, is a process of collecting information and data for statistical analysis.

To put it simpler, a questionnaire is a set of questions and only used to gather information. A survey is a process of collecting and analyzing the data. However, there is a questionnaire that is used in surveys, it’s called a survey questionnaire where the advantages of using questionnaire and surveys are combined.

Advantages and Disadvantages of Questionnaires?


  • Questionnaires are more practical.
  • A large quantity of data and information can be collected from a questionnaire in a short period of time and in a most cost-efficient way.
  • The validity and reliability of the questionnaire will not be affected even if many people will use it.
  • The result of the questionnaire can be acquired more quickly and effectively.
  • The questionnaires can be analyzed more scientifically and objectively than any other forms of research questionnaire and instrument.
  • The results of the questionnaire can be used to compare other data and can be used to measure change.
  • The result of the questionnaire can also be used to develop new theories or test existing hypotheses.


  • The questionnaire itself may lack validation.
  • Questionnaires can not fully capture some forms of information. Such as emotion, behavior, feelings, etc.
  • The questionnaire only asks the limited amount of information without the support of explanation.
  • It’s hard to identify if the respondent is being honest in answering and it is hard to tell how much thought a respondent has put into answering the questionnaire.
  • The respondent may not think twice of providing an answer to the questionnaire and may answer without a full understanding the questions.
