A donation is an act of someone giving a certain thing. May it be monetary or a thing to a non-profit organization or a charity.
The difference between sponsorship and donation is that sponsorship is a goal to aim for a certain business platform. Donations however support a cause without the business platforms. The funds that are given are merely given for no hidden reason.

For anyone who wishes to sponsor for an event, they must participate to do that. It is a requirement to participate if you want to sponsor.
In some donation forms, it is not necessary to fill up your contact information. But for sponsoring in an event, they may ask everyone to fill up every detail in the form. You may ask the person in charge if you still wish not to fill that up. But in most cases, you must fill it up as well, as they need the information.
