Consent is a permission or an agreement which can be both written and oral, to do something before you are able to do the action. The most common consent is usually a written consent. Consents come in different forms.

A consent form is needed for a scholarship to assure the parents/guardians that their child is given the free will to accept or decline the scholarship. Granting that both parties are okay with the idea, they have to consent.
