An incident report is a document which reports the events during a specific incident. It provides a detailed description of what actually happened upon the occurrence of the incident. Incident reports are usually written by witnesses and/or the people involved in the incident.

What to Include in an Incident Report
Like other reports in pdf format, incident reports need to contain accurate facts and information regarding and incident or an accident that recently took place.
Here are the basic things to include in an incident report:
- A detailed description or narrative of the incident.
- An even more detailed description of casualty, injury, and/or damage to property (if any).
- Date, time, and location of the incident.
- Accounts from witnesses, and other people involved in the incident, including their names, address, and contact details for further clarifications.
- Photos or snapshots of the incident.
- Probable contributing factors of the incident.
- Brief notes from the investigating officer.
- Suggested preventive measures to reduce the possibility of another incident.
Incident reports vary from one place to another. Some might include things not listed above, and some might not include things found above. Still, note that one must always remember not to include biased opinions in incident report writing.
Difference between an Incident and an Accident
We’ve mentioned these terms countless times before, and I’m sure you know both terms are often used interchangeably and sometimes even used to mean the same thing.
Let’s take time to know their difference from each other:
- An incident technical report refers to an unforeseen event that happens at a certain time and can refer to both positive and negative occurrences. Incidents vary from minor incidents (those that cause little or no injury or damage), to major ones (those that cause injury or damage).
- An accident is still an unforeseen event but with a more negative implication as compared to the word incident. Accidents also cause injury, damage, or even casualty.
Incidents refer to general unplanned occurrences, and accidents refer to specific kinds of incident. Basically, an accident can be considered an incident (or a type of incident), but an incident cannot be considered an accident.

How to Write an Incident Report
A good incident report gives reliable information without offending the readers or the individuals involved in the incident.
Here’s how you write an incident report:
- Collect and record the facts. Investigate immediately. Interview witnesses, and the persons involved. Know everything there is to know about the incident investigation report.
- Make a timeline of the incident. Connect the dots, find the missing puzzle pieces. Imagine the scenario and write it down in a detailed description.
- Be accurate, specific, clear, and honest. Don’t write something you’re not sure of, do not use vague descriptions, use simple words, and do not tamper with the information.
- Interpret the facts, determine the possible causes of the incident, and other contributing factors.
- You may recommend further investigation of the incident.
- Review, or double check the information you presented and proofread your work.
