  • Put it in chronological order – As you begin to write your version of the story, remember to start from the beginning. It is clearer and more precise for your reader to understand what may have triggered the incident. As well as having to start at the beginning can make you think of the flow of how you remembered it happened.
  • Make your statements clear – It is highly understandable that you may want to tell exactly how it happened. That you forgot to make the statements clearer and more concise. Avoid having to add unnecessary information as this can lead to swaying away from the truth and from the reason behind the incident.
  • Narrate what you saw happened- Only tell the part that you saw happened. Do not put other unnecessary details that did not happen during the incident. This is pointless as your supervisor is only interested in what you have to say and how you saw it. This is also to make sure that what you saw is in line with what others may have seen.
  • Explain the nature of the incident – For your supervisor to pass judgment, he or she must understand the nature of the incident. This is also your chance to write it down and explain how you saw it. Was it by accident or was it on purpose? How did it happen? Who was there? These are simply guide questions in answering the nature of the incident found in the report form.
  • Sign the report and hand it to your supervisor –  Once you are done writing down your point of view of the incident, sign it. This is also to prove that what you have written down is clear, concise and true to your knowledge.
