A record of the events that took place during that time and place.

Definition of Hospital Incident Report

Hospital Incident Reports are just a few of the types of reports done to record details of accidents or incidents inside the workplace. Hospitals, nursing homes and healthcare workers are usually the types that fill out this incident report. As it is most common in workplaces such as those. These types of reports are mostly common around healthcare and hospitals. The reason for this is because events like patient injury, accidents and other common types of occurrences often happen in work places like these. Incident reports can also be made to support your case for what choice you use it for.

Tools for Incident Report

There are a lot of ways to write an incident report. As well as ways of filing for an incident report. Regardless of which one you are going to use, it will always be the same thing. You are using that tool to file for the report. So check out some of the common ones listed below.

  • Letter – Yes, you can write a letter to report an incident. This is still considered a tool or a way to write your incident report. It follows the same rule and format as any other letter. State your name, the reason for reporting, the address of the occurrence, the people involved and the steps that were taken to address it. Writing a letter is mostly used by people for a more personal reason when doing an incident report.
  • Questionnaire- Questionnaires are also used for an incident report. It is far more simple than writing a letter. As the only thing you are going to be doing is to write what is being asked of you. However, there would be some questions that may ask for your personal details. If you are not as keen on giving them out for the sole purpose of filing for an incident report, I highly suggest you find a different tool to use. You may like to try the next one.
  • Narrative- Unlike letters and questionnaires, narrative reports are a more specific type of tool to use for reporting incidents. Narratives simply go from general to specific as you narrate what had happened during those times of the incident. You may also include the times, the people involved when you write it down. Like any narrative, it also follows certain rules.
  • Outline- Lastly, we have the outline. Outlines are mostly being used in a way that includes steps to take. Outlines are used when you are planning on how to address the situation. If you are thinking about making a step by step report, I highly recommend using an outline. It would be easier to break down the incident into specific sub plots and look for ways to avoid the problem or to improve on it.
  • Forms- Forms are the most common tools when you want to report an incident. Some forms can range from simple and general to complex and very specific. When you fill out a form, make sure you have read and understood what is being asked. As some forms can be too difficult to understand as well as some questions may seem to be the same and still you are told to give the answer.
