ANNUAL SELF PERFORMANCE REVIEW
KEY ACCOMPLISHMENTS
Looking back over the past year on the medical-surgical floor, my primary focus has
been delivering safe, evidence-based patient care while adapting to higher acuity
patient loads. One of my most rewarding experiences was acting as a primary
preceptor for a new graduate nurse. Transitioning from the classroom to the floor is
incredibly overwhelming, so I focused on helping her build critical thinking skills
rather than just checking off tasks. By creating a structured daily feedback routine, I
helped her confidently manage a full patient assignment by the end of her orientation
period. Seeing her thrive independently has been a major highlight for me.
Clinically, I am proud of my participation in our unit's initiative to reduce hospital-
acquired infections. I took the initiative to strictly audit my own Foley catheter care and
central line dressing changes against our updated protocols. I also actively reminded
colleagues during bedside handoff to assess the necessity of indwelling lines daily. This
small shift in our shift-change communication contributed to our unit going over 90
days without a catheter-associated urinary tract infection (CAUTI).
I also successfully navigated several rapid response situations this year. In one specific
instance with a respiratory compromise patient, my early recognition of subtle vital
sign trending allowed us to intervene and transfer the patient to the ICU before a full
code blue was necessary, highlighting the importance of vigilant monitoring.
CORE COMPETENCIES & STRENGTHS
Patient Advocacy & Education:
I believe that empowering patients is just as important
as treating them. I consistently take the extra time to ensure my patients and their
families understand their discharge instructions, new medications, and warning signs
to watch for at home. I use the "teach-back" method extensively, which I feel directly
reduces our 30-day readmission risks.
